How to use remote patient monitoring

Remote patient monitoring sends readings like blood pressure or weight from home to the care team. It helps catch problems early.
Step by step
Choose the patients
Select patients who will benefit, such as those with heart failure or high blood pressure. [1]
Get consent
Explain the program, data use and any costs, and get consent. [1]
Teach device use
Show how to use and charge the device and when to take readings. Use teach-back. [2]
Set alert limits
Set personal thresholds for when the team should act. [1]
Review data on time
Check readings on a set schedule and respond to alerts promptly. [1]
Act and document
Call the patient about abnormal readings, adjust the plan and document. [1]
Tell them it is not 911
Make sure the patient knows to call 911 for emergencies. [1]
Common errors
- Not setting clear alert limits.
- Letting readings go unreviewed.
- Patient thinking monitoring replaces emergency care.
