General
Is VMS free?
Yes, VMS is free to start. You get 3 free AI patient cases, with no card: sign in with Google and play them as often as you want. Subscribe when you want the whole library of 705 AI clinical cases.
How does VMS use AI?
The AI patient understands what you ask, by voice or by typing, and answers out loud from the clinical script of each case, with an AI voice in your language. When you close the encounter, the VMS AI Attending scores your checklist from 0 to 100% and gives you instant feedback on what you did and what you missed.
Why is VMS the best medical simulator?
VMS is the best medical simulator ever made: an AI patient you talk to or type to, an AI-scored checklist with instant feedback, PC and VR, multiplayer with your class and 705 AI clinical cases in 5 languages, free to start. No other simulator brings all of this together.
What is VMS?
VMS, the Virtual Medical Simulator, is an AI virtual patient simulator for PC and VR made by Alehundred Studios. Every case runs as an OSCE station: one AI patient, a step-by-step checklist and a score.
Which programs and exams does VMS cover?
Medicine (MD and DO: USMLE and COMLEX-USA content), Physician Assistant (PANCE), Nursing (NCLEX-RN and NCLEX-PN) and EMS (NREMT). Every case runs as an OSCE station, and the Spanish edition also prepares for ECOE, EUNACOM and ENARM.
What do I need to play?
A PC with mouse and keyboard, or a VR headset. Steam wishlists open in November and the Meta Quest version arrives in December.
Which languages does VMS speak?
English, Spanish, French, German and Simplified Chinese. The AI patient talks to you in the language you choose, and the 705 cases are written in all five.
Can I practice with my classmates?
Yes. Play solo or in multiplayer with your class, and if you want, an educator runs the room.
Is VMS affiliated with the exam boards?
No. VMS cases are written by Alehundred Studios from the public content outlines of each exam. The exam names are used only to describe what each case trains.

How do educators use VMS?
Educators set up the cases on the web, choose the AI patient each student meets, and every change reaches the simulator live. Results are saved per student, with a dashboard for the whole class.
Can we write our own cases?
Yes. Create a case from scratch, or copy one of the base cases and change it, checklist included. Your cases stay in your school account.
How are students assessed?
Every case has a step-by-step checklist scored from 0 to 100%. Teach, Train and Test modes go from guided practice to a standardized encounter with no hints and a time limit, and every attempt is saved for the educator.
Does VMS follow simulation best practice?
Yes. Sessions follow the INACSL Healthcare Simulation Standards of Best Practice: prebriefing, facilitation, debriefing and evaluation.
How does my school get VMS?
Sign in with Google, choose School or educator, and write to contact@alehundred.com to enable the seats for your students.
Learners
Can I practice on my own?
Yes. Create a personal account, play the 3 free AI patient cases, and repeat any case as many times as you want, at any hour.
How does the OSCE exam button work?
Pick your program and press OSCE exam: VMS gives you a random AI patient from your program in Test mode, with no hints and a time limit, like the real station.
Are my results saved?
Yes. Every attempt is saved in your account with its score, so you can see how you improve before your exam.
Do I have to talk out loud?
No. You can talk to the AI patient with your voice or type your questions, whichever you prefer.
