How to triage a patient by phone

Phone triage decides how fast a patient needs care. Missing a red flag can delay emergency treatment.
Step by step
Identify and locate
Get the patient's name, date of birth, callback number and current location. [1]
Ask about emergencies first
Ask about chest pain, trouble breathing, stroke signs, severe bleeding or confusion. [1]
Use a protocol
Follow your organization's triage protocol for the main symptom. [1]
Talk to the patient
Speak to the patient directly when possible, not only a family member. [1]
Decide urgency
Choose 911, emergency department, same-day visit or home care. [1]
Give safety-net advice
Tell the patient what signs mean they should call back or call 911. [2]
Document the care
Record the symptoms, advice given and the patient's agreement. [1]
Common errors
- Not getting the patient's location.
- Skipping red flag questions.
- No clear advice on when to call back.
