How to interview a patient who is short of breath

Shortness of breath comes from the lungs, the heart, the blood or anxiety. A focused interview tells them apart, but first make sure the patient can breathe while you talk.

Step by step

  1. Check how sick the patient is

    Can they speak full sentences? Look for blue lips, use of neck muscles and confusion. If present, sit them up, give oxygen per protocol and call for help first. [1]

  2. Introduce yourself and get consent

    Keep it short and let the patient answer with few words if speaking is hard. [2]

  3. Onset and course

    Sudden (minutes) or gradual (days, weeks)? Constant or in episodes? At rest or only with effort, and how far can they walk now compared with before? [3]

  4. Position and night symptoms

    Worse lying flat (how many pillows)? Waking up gasping at night? Both point to the heart. [3]

  5. Associated symptoms

    Cough and sputum (color, blood), wheeze, chest pain, fever, palpitations, leg swelling, weight gain. [1]

  6. Risk factors and history

    Asthma or COPD (past hospital stays, intubation), heart failure, clots, recent surgery or travel, smoking in pack-years, work exposures and allergies. [1]

  7. Medications

    Inhalers and how often they are used now, diuretics, new drugs (for example beta-blockers), and if they ran out of anything. [1]

  8. Summarize and plan

    Say the most likely cause, the dangerous ones you still need to rule out and the next steps. [2]
