How to set up cardiac monitoring (3- and 5-lead)

Correct placement gives clean tracings and fewer false alarms.
Step by step
Skin
Clean, dry, clip hair; fresh electrodes. [1]
3-lead
White under the right collarbone, black under the left collarbone, red on the left lower chest. [1]
5-lead
Add green on the right lower chest and brown at the V1 position (or as needed). [1]
Choose a lead
Lead II for rhythm; one with clear P waves. [1]
Alarms
Set limits for this patient; never turn them off. [2]
Change
Electrodes daily or per policy. [1]
Common errors
- Electrodes over bone.
- Default alarms for everyone.
- Old electrodes.
Practice it in VMS
