How to read and check a patient wristband

Other platforms explain it with text and video. In VMS you practice it on the patient.

Nursing (NCLEX)

The wristband links the patient to the right orders, medicines and samples. A quick, careful check prevents wrong-patient errors.

Step by step

  1. Check it is present

    Every admitted patient needs a wristband. Replace a missing or unreadable one before any care. [1]

  2. Ask the patient

    Have the patient say their full name and date of birth. [1]

  3. Compare to the band

    Match what the patient says with the wristband, letter by letter. [1]

  4. Compare to the order

    Match the band with the order, label or medicine record. [1]

  5. Look at alert bands

    Check for allergy, fall risk or limb restriction bands. Follow your facility's color system. [2]

  6. Scan the barcode

    Scan the band when your system uses barcodes. Never scan a copy taped elsewhere. [1]

  7. Stop on a mismatch

    If anything does not match, stop and fix it before giving care. [1]
