How to read a BNP result

BNP and NT-proBNP are released when the heart walls are stretched. They help tell heart failure apart from other causes of breathlessness.

Step by step

  1. Know which test you have

    BNP and NT-proBNP are different tests with different numbers. Never compare one with the other. [1]

  2. Use the rule-out cut-offs

    In sudden breathlessness, BNP below 100 pg/mL or NT-proBNP below 300 pg/mL makes heart failure unlikely. [2]

  3. Adjust NT-proBNP for age

    Rule-in cut-offs rise with age: about 450 under 50, 900 from 50 to 75 and 1800 pg/mL over 75. [2]

  4. Think of other causes

    Kidney disease, older age, atrial fibrillation, pulmonary embolism, sepsis and lung disease can also raise the level. [2]

  5. Watch for falsely low values

    Obesity lowers these peptides. A normal level in a patient with obesity does not fully exclude heart failure. [2]

  6. Check for sacubitril-valsartan

    This drug raises BNP but not NT-proBNP. Use NT-proBNP to follow patients who take it. [3]

  7. Link to the patient

    Combine the result with the exam, chest X-ray and echocardiogram. [1]
