How to interview a patient who is vomiting blood

Vomiting blood can be life-threatening. Stability and help come before the details.
Step by step
Stability first
Pulse, blood pressure, pallor, dizziness. Call for help and plan IV access if unstable. [1]
Introduce yourself and get consent
Keep it short if the patient is unwell. [2]
Amount and color
Streaks, cupfuls, bright red or coffee grounds. [3]
What came before
Heavy retching before the blood suggests a tear in the esophagus. [3]
Black stools and fainting
Black tarry stool and fainting mean a significant bleed. [1]
Risk factors
Alcohol, liver disease, past ulcers, anti-inflammatory drugs, aspirin, steroids, blood thinners. [1]
Summarize and act
Explain the urgent next steps. [2]

- Long interview before checking stability.
- Not asking about liver disease (varices).
- Missing blood thinners.
