How to give discharge teaching
Other platforms explain it with text and video. In VMS you practice it on the patient.
Many readmissions start with a patient who left unsure of the plan. Good discharge teaching is short, clear and checked.
Step by step
Start early
Begin teaching soon after admission, not at the door. [1]
Include the caregiver
Ask who helps at home and invite them to the teaching. [1]
Review every medicine
Go through each one: new, changed or stopped. Explain why and when to take it. [2]
Name the warning signs
Say which symptoms mean call the clinic and which mean call 911. [1]
Confirm follow-up
Give the date, time and place of appointments and any test results still pending. [1]
Keep it simple
Focus on 3 key points and give a written plan in plain language. [3]
Use teach-back
Ask the patient to explain the plan in their own words, then fix any gaps. [3]
Common errors
- Rushing the teaching at the last minute.
- Skipping the medicine review.
- Asking Do you understand? instead of using teach-back.
