How to explain heart failure

The name heart failure scares many patients. Clear teaching helps them spot early warning signs and stay out of the hospital.

Step by step

  1. Explain the name

    Heart failure means the heart does not pump or fill as well as it should. It does not mean the heart has stopped. [1]

  2. Explain the symptoms

    Fluid backs up, causing breathlessness, swollen legs and tiredness. [2]

  3. Teach daily weights

    Weigh every morning after using the toilet, before eating, in the same clothes. Call if weight rises 2 to 3 pounds in a day or 5 pounds in a week. [3]

  4. Talk about salt and fluid

    Limit salt and follow any fluid limit the team gives. [2]

  5. Explain the medicines

    Several drugs work together to protect the heart and remove fluid. Take them every day, even when feeling well. [4]

  6. Name the warning signs

    More breathlessness, needing more pillows, new swelling or fast weight gain: call the clinic. Chest pain or severe breathlessness: call 911. [3]

  7. Use teach-back

    Ask the patient what they will do if their weight goes up. [1]
