How to assess frailty

Frailty means low reserve, so small stresses can cause big declines. Finding it helps plan safer treatment and support.
Step by step
Ask about weight loss
Ask about unplanned weight loss over the past year. [1]
Ask about energy
Ask if everyday tasks feel like a lot of effort most of the time. [1]
Test strength
Check grip strength or the ability to rise from a chair without using arms. [2]
Time the walk
Measure walking speed over a short distance. Slow speed is a key sign. [2]
Ask about activity
Ask how active the patient is in a typical week. [1]
Use a scale
Use a simple tool such as the Clinical Frailty Scale to rate the overall level. [1]
Plan support
Refer for exercise, nutrition and medicine review, and discuss goals of care. [2]
Common errors
- Judging frailty by age alone.
- Not timing the walk.
- Not linking frailty to the care plan.
